For checking your LEI application, it is necessary that we receive additional documents from you. Please use the following form to upload your documents.

If you are not a registered representative who has the individual power of representation or if this is not apparent from the documents provided, please send us the template for power of attorney that you will find attached signed by an authorized person or by jointly representatives.
In the following, you will find a list of documents that sufficiently authorize each kind of foundation.

Foundation under Civil Law- one of the following documents will be sufficient if the name of the foundation and the representative authority of the applicant/ authorizing person is clear.

foundation register extract from the state foundation supervision or rather from the respective federal state (the representative authority is regularly apparent from this document and is sufficient for our validation)
a document certifying the power of representation issued by the respective state government/ regional council (proves the representative authority, document is sufficient for our validation)
recognition certificate, approval or deed of foundation issued by the foundation supervision or rather state government/ regional council (representative authority is not always apparent)

Catholic Church Foundation- one of the following documents will be sufficient if the name of the foundation and the representative authority of the applicant/ authorizing person is clear.

foundation register extract from the state foundation supervision or rather from the respective federal state (the representative authority is regularly apparent from this document and is sufficient for our validation)
register extract from the church foundation supervision, as the case may be issued by Bishopric or rather Diocese (proves the representative authority; document is sufficient for our validation)
Certificate of recognition of the bishopric, the episcopal tax chamber or the diocese (this document does not always prove the representative authority)
certificate of appointment of the priest (in most cases this only reveals the representative authority of the priest since the name of the foundation is usually not named, this document is not enough for our validation)

Protestant Church Foundation- one of the following documents will be sufficient if the name of the foundation and the representative authority of the applicant/ authorizing person is clear.

foundation register extract from the state foundation supervision or rather from the respective federal state (the representative authority is regularly apparent from this document and is sufficient for our validation)
deed of foundation of the respective church authority (the representative authority is regularly apparent from this document and is sufficient for our validation)oundation of the respective church authority (the representative authority is regularly apparent from this document and is sufficient for our validation)
